LOUISVILLE, Ky. — The Mayfield candle factory, where several people died during the deadly December tornadoes, is facing seven violations from the U.S. Occupational Safety and Health Administration (OSHA), which includes a lack of emergency plans.
In the days after the tornado, a four-day search and rescue operation was underway at the site. First responders from all over the state, including Louisville, were there working around the clock until every person was accounted for.
Some of the other OSHA violations include improper maintenance, safeguards and operational features for exit routes.
Two of the citations cite a lack of emergency action plans and four violations for bloodborne pathogens.
All of the violations are classified as “serious” and the fines total $40,000.
Mayfield Consumer Products has contested all seven, according to information on OSHA's website.
The investigation report also mentioned six employees were killed “due to multiple blunt force trauma injuries – after manufacturing building collapses due to tornado."
An expansion by the company was previously announced at its factory in nearby Hickory, where it plans to invest $33 million and employ more than 500 people in the next five years.
The case is still open.
